vancomycine dosering: oplaaddosis 20 mg/kg, vervolgens 30 mg/kg continu per 24 uur. Onderhoudsdosering vancomycine verder afhankelijk van nierfunctie en van gemeten spiegels, zie bij vancomycine en bij TDM vancomycine
