Volwassenen
creatinineklaring 30-50 ml/min: 2000 mg oplaaddosis, gevolgd door 2dd 2000 mg of 4000 mg continue infusie;
creatinineklaring 10-30 ml/min: 2000 mg oplaaddosis, gevolgd door 2dd 1000 mg of 2000 mg continue infusie;
creatinineklaring kleiner dan 10 ml/min: 2000 mg oplaaddosis, gevolgd door 1dd 1000 mg of 1000 mg continue infusie.
Renal function-replacement therapy
CAPD: 2000 mg oplaaddosis, gevolgd door 2dd 500 mg. Bij peri-operatieve profylaxe gedurende 24 uur is aanpassen van de dosering niet nodig.
Hemo dialysis: 2000 mg na dialyse wanneer het interval tussen dialysesessies 48 uur is, 3000 mg na dialyse wanneer het interval tussen 2 dialysesessies 72 uur is. Bij peri-operatieve profylaxe gedurende 24 uur is aanpassen van de dosering niet nodig.
CAV / VVHD: Continue venoveneuze hemodialyse/hemo(dia)filtratie: 2000 mg oplaaddosis, gevolgd door normale aanvangsdosis, gevolgd door 2dd 2000 mg. Bij peri-operatieve profylaxe gedurende 24 uur is aanpassen van de dosering niet nodig.
